What we do?
Incident Investigations
Whats Involved?
A qualified investigator will work closely with you to conduct an incident investigation. An investigator will conduct a comprehensive analysis and investigation to identify contributing factors, causal factors, hazards, conditions, actions, analysis and possible improvements.
An investigator can provide education and guidance for the business to better manage and respond to incidents ranging from a minor to critical nature. We only engage in incidents related to occupational health and safety.
Our Considerations?
When conducting investigations, we consider a multi-faceted approach considering brand reputation, people, operational systems, cost, legal and safety elements to mitigate potential harm.

Benefits
Reporting and investigating safety incidents is an important part of managing health and safety in the workplace. We’re independent workplace incident investigators, ICAM (Incident, Cause and Analysis Method) accredited. The ICAM system is a high benchmark of workplace incident investigations. It focuses on examining all factors that led to an incident rather than finding fault with a single factor. The ICAM system recognises that even with human error, many other issues may contribute to an incident.
Outcomes and Results
Engaging an independent investigator as soon as possible is critical to the effectiveness of the investigation and reducing further legal and workplace harm. We’re trained to identify ways to reduce the risk of litigation and criminal charges.
As part of the investigation we make recommendations to address hazards and exposures, often in the form of a corrective action plan such as retraining, management meetings, modifying safety systems, or new safety protocols. We will provide this information to you confidentially in a detailed report.
In addition, legislative compliance can be embedded into existing risk assessments that demonstrates compliance, due dilligence and strengthen risk justification as part of incident management.
